EDUCATIONAL AND LEARNING OBJECTIVES

Upon completion of this activity, participants will be able to:

  • Explain how sensor-based glucose monitoring can be used to improve health metrics in persons with diabetes and obesity
  • Describe new device-based advances in CGM technology, including how to use Glucose Pattern Insight Reports (GPIR) to guide both patient-based behavioral change and clinician-directed interventions in persons with diabetes
  • Discuss the critical importance of continuous glucose monitoring to ensure attainment of glycemic targets—TIR, GV, and HA1c/GMI—in obese patients with Type 2 diabetes, regardless of therapeutic interventions used to lower blood glucose
  • Identify the processes, triggers, and authorization protocols for reimbursement for technologies/devices used for continuous glucose monitoring
  • Illustrate the practice efficiencies and patient education/behavior guidance dimensions of using the Glucose Patterns Insight Report available on some CGM devices
  • Identify a broad range of persons with Type 2 diabetes, including those on oral therapies for obesity, who can potentially benefit from flash continuous monitoring
  • Establish protocols and clinical decision trees for incorporating and implementing sensor-based CGM within the management workflow for diabetes care in the physician associate and primary care settings

This Clinical Excellence Summit is directed at the following: nurse practitioners, physician associates, diabetes and endocrinology specialists and researchers and related healthcare providers who manage patients with Type 2 or Type 1 diabetes.
